ments also gives you total control of the
procedure, helping you create optimal
contours and esthetics.
This article features the restoration of
tooth #9 with a custom abutment and
e.max CAD crown using the new abut-
ment design of the 4.2 software.

An Astra Tech Osseospeed 4.5
implant was placed in the #8 position by a
local oral surgeonwith excellent primary
stability. He placed the healing cap and the
patient presented inmy office the same day
for an immediate provisional crown.

A Lava Ultimate block was used
to create a screw-retained immediate
provisional on a stock 6 mm titanium base
(Glidewell). The restoration was taken out
of occlusion both inMIP and excursive
movements. Care was taken
to create gingival contours
that help the tissue heal in
the correct location. This
one of the exciting new features
of the CEREC V4.2 software is the ability
to create custom abutments. This applica-
tion turns a procedure that has tradition-
ally been a multi-appointment affair with
high lab fees into a potentially one-visit
and inexpensive appointment. Economics

creates more predictable final results, and
makes the fabrication of the final restora-
tion easier.

After five months of healing,
the patient presents for a final tissue
check before moving forward with the
final abutment and crown. Notice how
the gingival zenith and papilla healed
symmetrically to tooth #9.

Upon removing the provisional
restoration, the tissue healed fantastically,
with a profile that mimics the natural
gingival contours. This will make the
final restoration fabrication predictable
by following the contours created by the
screw-retained provisional.
It’s important to note that you should
not proceed with a final restoration until
the tissue is fully healed. Rushing the
procedure can result in possible recession
of the tissue by placing too much force on
the gingiva.

Abutments are now available as a single-
tooth option in Restoration Types. You
can see that we did this case in Biocopy off
the provisional restoration and used the
Split (Multilayer) function. This allows
