After drawing the Biocopy line,
you are able to go into the Design phase
and design your restoration. The software
will ditch out the model underneath the
clinical crown, allowing you to customize
your emergence profile based on the tissue
(Gingivamask). The red dot in the crown
signifies the placement of the screw access
for the custom abutment.

After finalizing the design of
your restoration, you have the ability to
“split” the crown into a custom abutment.
In this step, you can modify the margins
of the abutment in relation to the tissue
(Gingivamask is slightly transparent in
this image). This allows you complete
control of margin placement and emer-
gence profile. Furthermore, the software
will remember what it splits away. That
is, one design will produce both your
custom abutment and your crown.

In this case, a soft-tissue
model was created to help with the final
contouring of the restoration. This image
shows the milled and sintered zirconia
abutment bonded to the TiBase and
seated on the model. Notice the precise
fit between the two.

Here, the final abutment is
torqued into the implant. You can see
some of the TiBase showing through the
zirconia abutment on the distal-facial
due to the slight lingual inclination of
the abutment axis (Fig. 15). When using
e.max CAD LT, there are very few
problems blocking out this discoloration.
A radiograph of the abutment is also
taken to verify that the complex is
seated correctly.

Creating custom abutments has brought
yet another innovative feature to the
chairside CEREC software. Having total
control of not only the crown design,
but also the contours and emergence
of the abutment, will allow CEREC
dentist everywhere the ability to restore
implants with predictability and effi-
ciency … and all for the cost of a stock
abutment.
