After implant placement, the Strau-
mann 4.1 Sirona ScanPost and ScanBody
was placed and imaged (Fig. 15). For a
provisional restoration, we are going to
design a one-piece, screw-retained unit
that can be bonded to the Sirona tita-
nium base extraorally. The design itself
will again be done via Biocopy. We can
scan the duplicated wax-up model and
easily stitch the model to the intraoral
scans. This will ensure that our final
design has the tooth position in the
original planned position (Fig. 16). The
CEREC software also has the ability to
overlay the gingival tissue to evaluate
emergence profile and tissue pressure
of the provisional restoration and also
allows us to overlay the Biocopy scan of
the wax-up (Fig. 17). As can be seen,
the original design was shortened
from the original wax-up position
enough to take the restoration out
of occlusion and minimize imme-
diate loading of the implant. The
final design is then milled with the
new Ivoclar Vivadent Telio CAD
PMMA blocks (Fig. 18). The final
restoration was bonded to the tita-
nium base using Multilink Hybrid
Abutment HO, polished and screwed
into the implant with strong finger pres-
sure. The screwaccess holewas restored
with composite.
The provisional restoration immedi-
ately improved the cosmetics of her smile
and, most importantly, began the process
of adapting and healing the tissue for
the final restoration in a few months.
You can see this in Figures 19-21 (please
note that these pictures were taken three
weeks after the live surgery, and also note
that, with the patient’s permission, we
rounded off the opposing canine in order
to reduce the stress on both the imme-
diate and final restoration).
The patient was extremely pleased
with the experience and result, and is
also very thankful for the immediate
improvement to her smile.

Immediate provision-
alization can be quite
difficult
and
time-
consuming. But, with
proper planning comes
proper execution. And
with Sirona’s digital solu-
tions andmanufacturers coming out with
new and better materials to complement
these solutions, these procedures will be
much more efficient and predictable for
both the clinician and patient.
