How do you restore your peg lateral cases?
What materials have you had most success with?
Prep versus no prep?
I have a case I am planning on a teenager completing ortho
now and would love to see pics of other peg CEREC cases.

I do not have any photos. The only thing I can
tell you is I am a Bluecam user with 4.2 software,
and I have great success with Biogeneric design when doing
anteriors. I would suggest minimal prep that has a shoulder
with at least 1.0 mm space for porcelain, and I would use
e.max making sure there is enough porcelain thickness on
the rest of the tooth for the strength of porcelain. I then stain
and glaze to the best of my ability to get shade match. If the
porcelain would look good without stain, then I do not stain.
Good luck. I also scan with the tip of the scanner pointing
toward the lingual using small steps with my Bluecam. I do
not know how to do this with Omnicam.

I rarely use a no-prep approach to peg laterals, I
use a minimal prep approach placing necessary
margins. I like using e.max and Empress Multi, and neither
of these mill well without a solid margin that prevents chip-
ping. I start these cases by directly bonding composite to the
laterals and contouring as perfect as possible, then letting the
patient walk for a week or two, checking function/occlusion
and esthetics. At that point, it turns into a rather simplistic
Biocopy case.

Karen, I have done my last few with hybrids
when on kids. I can do it basically no prep and
also, if they chip them (as all kids tend to do) I can repair
with composite. Done about four and all are holding up well.
Three with LAVA Ultimate and one with Cerasmart.

Do you give specifics to the orthodontist on how to posi-
tion the peg laterals prior to restorative tx?
Bob — case looks beautiful! How old was this patient? Any
chance you have the .rst file too? Would be interested in the
margination and design.
Is there any reason/rationale to wait until patient is done
growing to place ceramic restoration (i.e. e.max)?
