IMPLANT ANGLE
The angle of the implant determines if
a screw-retained restoration or split
file is needed. If the access hole is in an
unesthetic area, then the case should
be split. Also, if the ability to seat the
crown into the implant is difficult due to
the angulation, the case should be split
to enable seating of the abutment at an
off-angle, but with a more ideal crown
insertion (Fig. 1).

AMOUNT OF SOFT TISSUE PRESENT
Minimal soft tissue may leave insufficient
room for splitting the case, causing the
abutment interface and crown to show.
Also, if the TiBase is at or above the tissue,
a gingival mask should not be used. Do
not take one at acquisition, since it makes
creating a good emergence profile impos-
sible. If presented with a case with inad-
equate soft tissue, place a screw-retained
restoration and do not use the gingival
mask (Figs. 2-4).

RESTORATION INSERTION AXIS
The restoration insertion axis determines
the angle at which the abutment is split.
Presented with an off-angle
implant, a short abutment
could result if the axis is not
corrected (Figs. 5-6).

are CAD/CAM-fabricated, implant-
supported hybrid restorations for single
teeth. These hybrid restorations are indi-
vidually fabricated fromlithiumdisilicate
blocks and incorporate a TiBase. Two
different implant solutions are possible:
IPS e.max CAD hybrid abutment and
separate IPS e.max CAD crown, and IPS
e.max CAD hybrid abutment crown.
Following the tips below will help to
ensure success with individual CAD/
CAM-fabricated hybrid restorations for
implants—whether for single tooth resto-
rations in the anterior or posterior regions.

SPRUE LOCATION
Previously, when fabricating a hybrid
abutment, the laboratory would remove
the sprue — even if it was in a bad posi-
tion. Now, when creating a hybrid abut-
ment, care must be taken regarding
where the sprue is located (Figs. 7-8).

MODEL AXIS
The model access for
posterior teethshould
still be set based on
the plane of occlu-
sion to provide the
best proposal, even
if the implant is at an
off-angle. If aligned
with the implant instead, the proposal
will be at a skewed angle and requiremore
adjustments and design time. The implant
alignment may warrant splitting the file
in order to make a telescoping abutment
if the proximal contacts prevent a screw-
retained crown from seating. The smaller
abutment diameter will be able to be
seated past the adjacent contacts. (Fig. 9).
